Medicare does NOT pay for this service; this is a private pay service. However, some Long-term Care policies and employee assistance programs may reimburse some of the costs. The rate fee is usually charged using an hourly fee structure.

A good time to use a case manager is when the family starts seeing:

• changes in behavior
• weight loss or gain
• getting lost while driving or unsafe driving
• confused about finances
• when a spouse is overwhelmed as the caregiver
• there has been a new or complex diagnosis
• there is limited or no family support
• the problems are becoming complex and there is conflict within the family.

Using a case manager who knows what the available resources are would be beneficial.
