About Nicole

Nicole has her Master’s degree from the University of North Texas in Long-Term Care, Senior Housing and Aging and a graduate certificate in Applied Gerontology. Assisting her grandparents with navigating through chronic conditions she recognized the need for advocates for the elderly. She has been a resident advocate in nursing homes as a volunteer Ombudsman and a Certified Benefits Counselor through the Department of Aging and Disability. Nicole has also volunteered her time with Meals-on-Wheels, military organizations, the Alzheimer’s Association, and currently is a member of the Patient Experience Committee at Lewisville Medical Center. Additionally, Nicole has attained certifications as a CMC, Care Manager, Certified and also a CDP, Certified Dementia Practitioner.

Nicole enjoys spending her time with her husband of 20+ years and her family of three boys. She has enjoyed traveling the world with them while her husband was in the military. His military career provided her with the opportunity to connect with veterans and their families, and continues to allow her to assist them in navigating the complexity of medical and government resources.

 

Nicole's Story

  • 2008-2010

    The first time I went to visit my grandfather in a nursing home I saw him wearing women's capri pants that wouldn't zip up and worst of all, no one seemed to care. Unfortunately, being wheelchair bound and without someone there on a regular basis to help, this type of situation became the norm. I witnessed bloody nails, I witnessed bedsores, I witnessed personal items and money stolen. I never want to see what happened to my grandparents happen to anyone else's relatives.  The pain and confusion that my family went through was more than we could handle.

  • 2010-2011

    I started in this business journey after assisting my three grandparents with normal aging and Alzheimer’s. I was the oldest grandchild and always very close with my grandparents. During this time, I experienced the differences in care and oversight in both memory care and skilled nursing homes and what happens to those who don’t have someone to advocate for them.

  • 2011-2014

    My grandparents passed and I realized I had the desire to help other families who were going through the same process. It is stressful and complicated, and without someone there who knows who to ask and what to ask, it is easy to get lost in the system. So, I went to school to get my Master's in Long-Term Care, Senior Housing and Aging with a certificate in Applied Gerontology.

  • 2014-2017

    I interned with the North Central Texas Area Agency on Aging and became a volunteer Ombudsman and Certified Benefits Counselor during this time. Afterwards, I started working for as a care manager for a company based in Austin and gained a wealth of experience.

  • 2017-Present

    After recognizing the increasing need for care management in the North Texas area, I founded AgeWell Care Management. Helping families is a blessing and I assure you that I will advocate for you and your family to ensure that you receive the best possible care available.  I know this is a complicated and overwhelming process, I know it can be confusing and I want to be there to help guide you.

In the final years of her mother's life, Barbara Fairfield enlisted the help of a geriatric care manager.

Now Fairfield, 76, is planning to hire one again -- this time for herself, anticipating a time she'll need assistance. Caring for a senior "is not something you should do without professional guidance," she says.

For seniors and their families navigating the complicated maze of care, geriatric care managers can be a lifesaver, says Courtney Smith, elder support program director for the Senior Source, a Dallas nonprofit agency.

"The senior care system is so big and there are so many different areas," she says. "A senior who's been in the hospital may need to go to rehab or to assisted living, or maybe the senior can go home but needs resources to live independently. It's all very intimidating". . . read more
